  •   Taking the time to assess "Risks"

    The amount that you rush is directly proportional to the amount of risk you will incur. We are part of an industry, that frequently faces delays, abrupt changes in schedules and then subsequently places pressure on its staff to meet those new demands.

    It happens quite frequently that circumstances force us to be delayed.It is also understandable that all of us will attempt to hurry up and get back "on schedule."

    The initial dilemma is that although a certain level of "rushing" can be exhilarating and rewarding as we move quickly through our procedures, there comes a point for all of us in which our rushing can, "task saturate"us and leave us open to making mistakes.

    The final dilemma is that we are unaware of exactly what that point is.The only way that we realize that we are beyond that point is after we become aware that we have committed an error.

    Hopefully, it will not be a critical mistake and the pilot, engineer, or other staff member will slow down before entering that area of risk.

    It may be impossible to pick up that second or third "hazard", if our minds are so clouded by that first "hazard" of "rushing".
  •   Anonymous Reporting

    There may come a time at which you observe something that you may feel is not "quite right". For various understandable reasons you may not want it known, that you had filed a "Safety Report", finding faults with behaviors or attitudes within the system. You feel may it may affect your employment with the company.

    Sometimes you may justify your reasons for not reporting in such an uncomfortable situation, is that maybe you are wrong.On the other hand, not "speaking up" may also cause serious injury, financial loss or even worse. Once there is an accident the facts will come out.

    In many of these cases, there were a few individuals who had known, of the potential hazardous actions and behaviors but did not say anything. It could be that the persons involved, are not aware of the "Hazard".

    The "accountable executive" values all employees input on safety, and he or she will respect all"anonymous reporting", from all employees. In the end your report may be found at fault because you did not see the whole picture, however, if your report was accurate, you may have saved the company a serious accident loss.

    To file anonymously, first log out. Log in user: anonymous Password: anonymous.Once entered click on "File an SMS Report". When the "Department" selection opens, check button: "Anonymous Report"
  •   Basic Reporting

    1. To file a basic report first "register",then enter the area;

    2. Make a selection to the appropriate "Department",eg.s Flight hazard -"Flight department".If all fields are not entered your report will not submit.

    3. Make an entry in every field "text area", and "check box". Enter n/a or na as appropriate.

    4. Browse and upload,photos, or documents as an attachment, then submit your "Safety Report".

    5. The "Reporter" will let you know an email has been sent to the "Department head". Expect to receive your "first" follow-up soon.

    6. If your "Safety Report" has not been actioned, within a set period, it will be "escalated" automatically to another "Department". You will be notified if your "Safety report" has been escalated. For every follow-up, move to another "Department", transfer to another staff member, and eventual closing of your "Safety Report" you will receive an e-mail.

    7. The "Accountable Executive" or his or her delegate oversee's all "Safety Reports" and may become involved in a follow-up to your "Safety Report".Only the "Accountable Executive" or his delegate may close a "Safety Report".

  •   Assessing Hazards

    1. A "Hazard" as defined by CCOHS, is any source of potential damage, harm or adverse health effects on something or someone under certain conditions.

    filereportIn order to give you a starting point we ask that every employee,review the "Hazard Identification and Assessment" and "Hazard Identification Checklist" articles in the "SMS Reference Information" area. (Located at the very bottom right of the "SMS Reporter").

    This area may be updated frequently by your companies "SMS Reporters" management staff, with attached comments so review this area, before you send in your "Safety Report".

    2. From the "Risk Matrix" find the intersecting area according to loss severity and probability. There are four colour coded risk areas. LOW RISK green, MEDIUM RISK yellow, HIGH RISK orange and URGENT red. When you are filling out your "Safety Report" there is a drop down. Enter the risk from the "Risk Matrix", that is located in these tutorials.

    3. Once you enter the "Hazard Risk" it alerts the "Department" by e-mail as to the risk that you have assessed using the "Risk Matrix".The "SMS Reporter", will colour code your "Safety Report" to highlight it for the Department to which it was sent.
